Wegovy

MEDICINE AS A STATE PROJECT

How China is turning Wegovy from a Western slimming luxury into a mass public health tool in just 24 months

WHEN OBESITY BECOMES A MATTER OF STATE

Beijing, 2023. In the offices of the National Health Commission, a projection sends a chill down the spine: 630 million Chinese people overweight or obese by 2050.

The current figures are already brutal: 400 million adults overweight or obese in 2021, more than the United States and India combined. Childhood obesity has exploded in a single generation, from 1.3% to 15.2% among boys. Chronic diseases account for 70% of mortality. If nothing changes, the health system implodes, productivity falls and social stability is threatened.

This is no longer simply a public health problem. It has become a matter of state.

The Healthy China 2030 plan, launched in 2016, had set a goal: cut premature mortality from chronic diseases by 30%. But by 2024, the curves were not bending fast enough. A massive acceleration was needed.

Three opportunities converged:

1. A technology the population already knew. The semaglutide molecule (Ozempic) was approved in China for diabetes as early as April 2021. Three years before the official approval of Wegovy (semaglutide’s brand dedicated to weight loss), Chinese women were already swapping tips on Xiaohongshu for getting Ozempic off-label, nicknamed “网红减肥神药” (“the miracle weight-loss drug”). When the NMPA approved Wegovy in June 2024, the government did not have to discover a treatment. It simply had to orchestrate its mass adoption.

2. Providential timing. The Chinese patent on semaglutide expires in March 2026, 5 to 6 years before Europe and the US.

3. Industrial capacity ready to go. 15 to 20 Chinese laboratories already have their generics in Phase III, ready to launch as soon as the patent expires.

Faced with this window, the government made a singular decision: to become the project manager of public health. It would itself coordinate regulation, approval, the rollout of clinics, doctor training, national marketing, and even social media activation. Pharmaceutical companies, foreign and local alike, became the beneficiaries and executors of a plan steered from the top.

Between June 2024 and March 2026, the Chinese state would transform a luxury drug until then reserved for Western elites in pursuit of slimness into a mass public health tool to prevent diabetes, cardiovascular disease and chronic illness.

24 months flat.

THE DEMONSTRATION: THREE SYNCHRONIZED SCENES

February 2024: The film. Jia Ling, a plump actress-director, releases 热辣滚烫 (YOLO). The story: an overweight woman discovers boxing, loses 50 kg and regains her dignity. The twist: Jia Ling REALLY lost 50 kilos for the role. The film makes the top 3 at the 2024 box office. A mass emotional trigger. Millions of Chinese people watch “one of their own” transform. If she can do it, why not me?

March-May 2024: The hashtag. On Douyin, “国家喊你减肥了” (“Your country is calling on you to lose weight”) goes viral. A semi-official campaign relayed by state media, fitness KOLs, health influencers and celebrities. Aspirational in tone, never guilt-inducing.

June 25-26, 2024: The approval. On June 25 at 6 p.m., the NMPA approves Wegovy. 15 hours later, 16 ministerial departments simultaneously launch the “Weight Management Years” (2024-2027). Clinics will open. Doctors will be trained. Nutritional guidelines are issued.

These were not three separate events. They were three acts of a single play orchestrated by the state. The film primes the emotion. The memes prime the engagement. The approval delivers the tool. The national campaign delivers the infrastructure.

THE STATE AS PROJECT MANAGER: A THREE-PART MECHANISM

PART 1: CULTURAL – The state steers the narrative

In the Western model, pharma companies do their own direct-to-consumer marketing. In China, that is prohibited. Instead, the state steers the narrative and pharma companies benefit from it without having to build it.

No official “ordered” YOLO. But the film came out at an almost TOO perfect moment. The patriotic hashtags on Douyin are co-designed with the cyberspace administration. Fitness KOLs receive “editorial suggestions”. Celebrities lead by example. Government cartoons saturate WeChat.

The result: when Wegovy landed in June, public opinion was already emotionally engaged, culturally primed by 3 years of off-label Ozempic use. Novo Nordisk did not have to spend a single yuan on consumer advertising. The state had done the groundwork.

PART 2: POLITICAL – The state mobilizes the apparatus

June 26, 2024: 16 ministries swing into action simultaneously. Not a steering committee. An operational command.

What was rolled out:

  • Multidisciplinary Weight Management Clinics (nutritionist + endocrinologist + psychologist + coach) in every major city by 2026
  • Fast-track training of thousands of doctors in prescribing GLP-1s
  • 371,000 km of fitness trails (+107% vs 2019) within the “15-minute fitness circles”
  • Mandatory nutritional guidelines for restaurants and canteens

The hidden objective: prepare the ground for the March 2026 generics. The state lays the tracks before the train arrives.

The typically Western narrative: “The Party is turning a health indicator into a civic duty. Where is the line between protection and coercion?”

The Chinese answer: “You spent 4 years debating the price of Wegovy. We are going to treat 100 million people. Who protects their citizens better?”

PART 3: INDUSTRIAL – The state orchestrates the pharma companies

March 2026: the patent expires. 15 generics arrive simultaneously. Frontrunner: Hangzhou Jiuyuan Gene (Huadong Medicine) with “Jiyoutai”. Entry price: 300-600 RMB/month (€40-80) versus 1,400 RMB/month for Wegovy in China.

2027: the government launches VBP (Volume-Based Procurement). Likely final price: 200-400 RMB (€25-55/month). That is ÷22 to ÷48 vs the US price (€1,200/month) and ÷5 to ÷13 vs Europe, and 5 years ahead of the Western patent expiry.

This is not dumping. It is state-driven industrial engineering: local production, economies of scale (400 million potential patients), timing dictated by the political calendar.

The role of foreign pharma companies? Novo Nordisk and Eli Lilly capture an 18-36-month premium window, then watch their share collapse. The role of Chinese pharma companies? They wait for the signal, the patent expiry, to deploy capacity potentially 10x that of Novo + Lilly combined. Exports to Asia, Africa and Latin America follow in 2027-2030. The sales pitch: “Same molecule as Wegovy, 10 times cheaper.”

2030: MISSION ACCOMPLISHED?

Projections: 50 to 100 million patients on GLP-1s. Reduction in type 2 diabetes incidence: -20 to -30%. Health system savings: USD 10-20 billion. Healthy China 2030 goal: probably achieved.

IF YOU ARE A FOREIGN DRUGMAKER IN CHINA

1. Your window: 18-36 months. Between approval and VBP, go in with an exit plan.

2. The state steers the narrative; you support it. No direct-to-consumer marketing. Align yourself with the cultural levers the state activates. To do so, understand what is at stake in the Healthy China 2030 plan.

3. Clinics are the battlefield. Train the doctors, install your digital protocols. Whoever controls patient support controls the molecule.

4. Sell outcomes, not pills. Offer: drug + remote monitoring platform + population-level KPIs.

5. Prepare for Chinese exports. Generics will flood Asia, Africa and Latin America from 2027. Become a partner or be swept aside.

THE FINAL WORD

A film, a meme, an approval. Three acts of a single play orchestrated by the Chinese state. China did not “react” to obesity. It engineered a solution by synchronizing culture, politics and industry. While Europe debated, China executed.

Final question: is this an exportable model or an authoritarian singularity? Both. The synchronization is exportable. The centralized decision-making, less so. But the real lesson: France has been debating a pharmaceutical “Buy European Act” for years. China did it in 24 months with a film, memes, and a state that appointed itself project manager of its own public health.

In 2030, when 100 million Chinese people have lost weight thanks to generics at €30/month, the West will still be wondering whether Wegovy should be reimbursed.
